GIST Treated with Gleevec


A 74yrs old male, status post resection of Gastrointestinal Stromal Tumor (GIST) and history of adjuvant Gleevec (Imatinib mesylate) therapy 3 years ago, was referred for PET-CT imaging because of interval increase in size of the right lower quadrant soft tissue density mass, seen on a follow up CT scan.

PET-CT showed intense FDG uptake in the mass (maximum SUV 12.4) and multiple smaller hypermetabolic lesions in the abdomen, consistent with neoplastic involvement with GIST. Gleevec therapy was restarted and the follow up PET-CT in 3 weeks demonstrated resolution of hypermetabolism in all lesions, suggesting good response to therapy . The SUV of the RLQ mass decreased to 2.5.

Synchronous Squamous Cell Cancers


A 64 yrs old male presented with an exophytic lesion on the right foot 4th interdigital web space, and was found to have SCC on punch biopsy. Staging CT scan showed a mass in the epiglottis, measuring 2.0 cm x 1.6 cm in size. Biopsy under direct laryngoscopy revealed SCC.

Whole body PET-CT showed focal hypermetabolism in the epiglottis (SUV 9.0) and focal hypermetabolism in a 5mm endobronchial soft tissue density at the trifurcation of the basal trunk of the left lower lobe bronchus (SUV 3.9). Endoscopic biopsy showed poorly differentiated SCC and led to the management of three synchronous primary SCCs.

Cancer of Unknown Origin


A 71 yrs old male was admitted with increasing shortness of breath and was found to have a large left side pleural effusion. Thoracentesis revealed an exudate with many RBCs and the cytology was negative. CT scan showed a large left pleural effusion, circumferential pleural thickening, and a posterior mediastinal mass. EUS guided FNA of the mediastinal mass revealed adenocarcinoma.

Whole body PET-CT showed: FDG avid left lung pleural thickening, hypermetabolic right paratracheal lymphadenopathy (SUV 5.1), and focal hypermetabolism in the thickened lower esophagus (SUV 15.7). EGD revealed esophageal adenocarcinoma. Incidentally seen on PET-CT is an intensely FDG avid (SUV 36.7) fluid density mass in the left scrotum, from inguinal herniation of the urinary bladder.
